Anthony Hansen Counselling is committed to providing you with quality services (by phone or zoom) to support you.
1. I understand that Anthony Hansen Counselling:
a. will document notes of our meetings and communications;
b. may exchange (after I give my consent) my personal or health information with third parties external to Anthony Hansen Counselling.
2. I am not obliged to agree to the exchange of any information with third parties but understand that sharing my information may assist in the provision of service or support.
3. My personal or health information will be treated as confidential and in a way which complies with the Privacy and Personal Information Protection Act 1998 (NSW) and/or the Health Records and Information Privacy Act 2002 (NSW), as well as Anthony Hansen Counselling's Privacy and Confidentiality policy.
4. I understand that Anthony Hansen Counselling may disclose my personal or health information without my consent in certain circumstances, including where:
a. the information is connected to proceedings before a court or tribunal, including in response to a subpoena or notice to produce;
b. Anthony Hansen Counselling reasonably believe the disclosure would prevent or lessen a serious and imminent threat to the life or health of a person (that is, me or a third person);
c. the information is reasonably necessary for law enforcement purposes;
d. Anthony Hansen Counselling is required to comply with mandatory reporting requirements.
5. I have a right to make a formal complaint if I am dissatisfied with the way in which my personal and/or health information is collected, stored, used or disclosed (for more information about how your personal information is kept, please refer to the Anthony Hansen Counselling Fact Sheet).
6. This acknowledgement remains in force until Anthony Hansen Counselling closes my file (unless I advise otherwise).
CANCELLATION POLICY
Your appointment at Anthony Hansen Counselling is reserved especially for you. We understand that sometimes the unexpected happens, and you will need to cancel your booking. We ask that you please provide as much notice as possible, so the appointment can be offered to someone else in need.
Please be advised that any request for cancellation or rescheduling should be submitted no later than 48 hours prior to the scheduled session. Rescheduled time and date will be subject to time slot availability.
It is important to note that payments for cancelled sessions within 48 hours are non-refundable. However, they are available for rescheduling. If you have any additional inquiries or concerns before your session, please do not hesitate to reach out.
Credit or debit card details are required to confirm your appointment.
These are securely collected using our online form and will be charged to your card if you provide less than 48 hours’ notice of a cancellation or do not attend your appointment. The full fee will be automatically charged to your card on the day of your scheduled appointment, unless other payment methods have been arranged.
If you have an appointment booked for a Monday, we ask that you please notify of any cancellations before 3pm on the Friday prior.
Medicare rebates are only available for face-to-face, telephone or telehealth services provided.
All outstanding consultation fees need to be paid prior to any further appointments being booked.
